The 2026 European Society of Cardiology (ESC) Heart Failure Guidelines represent an important evolution in the treatment of heart failure. 

For the first time, mineralocorticoid receptor antagonists (MRAs) are recognised as foundational medical therapy and receive a Class I, Level A recommendation for HFpEF, reinforcing the importance of mineralocorticoid receptor blockade in contemporary heart failure management.1

A New Foundation for Heart Failure Care 

The latest ESC guidelines position MRA therapy alongside other evidence-based treatments as part of a foundational approach to managing heart failure.
